Let’s go through my real-life experience using fish bone diagram as a civil engineer in the Mumbai Metro Line 7 project, we encountered an issue while testing one of the spans in the project line. Following instructions, the span required reconstruction, which resulted in a significant delay and a substantial loss of company funds during the handover process to MMRDA. To tackle this problem, a team was assembled to identify the core issue behind the delay and extra costs incurred. The team utilized a fishbone diagram analysis to determine the possible causes of each branch of the diagram. The factors identified included inadequate supervision, semi-skilled manpower, poor communication, low-quality materials, and inadequate quality checks before execution. By identifying these root causes, the team was able to address the issues and provide a report for that incident.
